Ben, depends on whether you use 60# or 40# using forty it is more bags i would say
4 for 60# and 6 for 40#

At a guess I would say about 4 or 5, 20kg bags if you're adding aggregate to the mix. A 12ft slab will also require reinforcing mesh or re-bar for some added strength to stop cracking.

80lb bags = 7

This is quikrete bags.
