That was decided more than 1000 years ago. Groups fought. Those that won declared themselves King or royality. Those that lost either were killed or became the common folk.

No one 'decided' anyone should be royalty. The monarchical system grew out of the old tribal system. In the tribe, certain individuals were regarded with respect by others, because they were either more knowledgeable, or more forceful, than their neighbours. When there was lots of land and only a few tribes, if disputes arose, the disaffected group could leave the main tribe and form their own tribe.
Eventually, the land became too crowded to support this, so the tribes came together in battle. And, as everyone knows, in gamesmanship or in warfare, you need to have a leader. That leader became the 'ruler' of the tribe, and when his tribe was victorious, he became the ruler of the other tribe as well, which was absorbed into his tribe. Over time, the smaller tribes were gobbled up by the bigger ones, and the guy at the top of the heap was king of the castle. For as long as the skirmishes continued, the warlord was the ruler of the tribe. At some point, when one warlord died, his son challenged another warlord for leadership of the tribe, and won. After that, he declared it to be an hereditary position, which would go to his son after he died.
Other warlords liked the idea, the the notion of an hereditary monarchy was born.
Do you like that explanation? I just made it up, but it sounds reasonable to me, and the real answer is that no one knows exactly how royalty came about, but this is as good a theory as any.

Royalty was established by those who conquered and ruled territories in ancient times. Their sons and daugthers inherited their throne. Thus, kings and queens came into being.

The most greedy and violent people rose to the top in their societies and, with their gang members, they were eventually able to control whole countries.
This is the origin of the present day royal families.

I would imagine that each region of our planet had a way in dictating who would rule. Probably through influence, wealth religious status or shear bullying, etc...
