There are several companies offering this service.
I personally use SKYPE, which does not have a 'use-by' date for any credit you buy, it is free to call the USA but costs to ring the UK. With SKYPE you can also buy a cheap-rate number so that people can ring you through your PC, and this comes with an answerphone facility. SYPE is also a free IM (instant message), Video-call and PC-2-PC call service.
get it here: www.skype.com
or VOIPCheap, which DOES have a 'use-by' date for bought credit, but allows you to ring major destinations such as the USA and UK for free.
get it here: www.voipcheap.co.uk
You will need some sort of speaker/microphone set-up on your PC - most people use headsets with built-in mics as they are more private, but you can use separate speakers/mic or bluetooth wire-free combined headsets.
If you want to use video calling, then you will also need a webcam.

You Can do it through Yahoo messenger as well. Just run a search on it. It is similar in cost and you are already signed up for Yahoo. You need a microphone and speakers to talk and then you are golden. Cost per minute are around 2-3 cents a minute within the U.S. PC to Land line and other reasonable rates internationally.

Skype are just one of the companies offering a program and network that you can do this on. I think tesco are offering a similar service to skype now.

I have just added this facility - thanks to BT TALK - I now pay a little over £20 per month and get free calls evenings and weekends.
I previously had Skype but have found it troublesome and expensive in comparison.
