You never change out ALL the water in any kind of pond, pool, aquarium, etc. There are good bacterias that are in the water. Only change out a little more then half, leaving some of the old behind. The fish can go into shock otherwise.

according to me u would have made one or some of the following mistakes:
did u drain all the old water from the pond.
did u fill the pond with tap water.
did u introduce the fish immediately after u filled tap water
does ur pond have a filter or an air pump

It could have just died (which means you didn't do anything wrong). Although all the carp species (which include koi) are pretty hardy, you may have had the pH or copper levels of the watter way to high, although if that were the case you probably would have had more than one dead fish.
I would guess it just died of natural causes (or at least something which you had no control over).

The following may be the reasons:-
*It mighte be ailing for a long time, It is a hard fish it may not just die.
*Do not wash aquarium with hard soap at all even if you did please wash thoroughly.
*Always sifon out dirt every week leaving 50% water.
* Every 3 week clean thoroughly.
*Do not over feed kois ,once in a day food which can be consumed within 5 min.

sometimes you have to condition the water with something.. maybe it was too much change in temperature. Remember putting gold fish in a plastic bag with water and letting them acclimate slowly to the water temp? I'd check with whoever you got the koi from.. They would know.. (how sad!!!)

I'm sorry you lost one of your fish. If you changed all the water in your pond, it was probably shock that killed it. You need to do it gradually, so that it still contains the things they need. I hope any others you have are OK.

did you add water conditioner in the pond before you added the water. you might want to go to petsmart and get your water tested. it is free.

Perhaps, the water temperature change was too extreme!
Fish can be very sensitive to it.

if you used tap water, it was probably the chlorine in the water that did it, or either the sudden change in temperature
