it's a slang term meaning painfully thin. take a look at the rails on the ground, used by railroads. they aren't wide. while these sections of rails can be in 5-15 feet in length, their thickness is quite small or thin. ergo, when people use the expression ' rail-thin' they mean quite skinny. it's an analogy that most people can have no problem in understanding. get it? got it? good!
