There are two ways to look at this. When they say "what" they could be asking if you were not paying attention, if you didn't understand, or if you are possibly mentally ill. Maybe they think that you are ignoring them.
The person asking if you are "stupid or what?", may have no patience, be a mean person, or be stupid themselves for putting it that way, instead of, "did you get that?"
It would really throw the person asking this, if the other person were to ask them your question!

"or what" is actually a window to explain ones actions that were thought to be "stupid" by another.

The speaker is indicating he believes you must be stupid to do what are doing, but is giving you a chance to explain if indeed there is any explanation to be offered.

Anyone who says something like this does not have any original thoughts or comments. This is a cliche' it is actually quite meaningless.

I think it's a hypocritical apologetic "what,' as in "you made me call you stupid because WHAT else is that behavior/question?"

people who use that line are stupid because it doesn't make any sense. they just try to sound cool!
