I'm looking for a form online, but can't find anything. I'm trying to finish up an application but I can't recall all the jobs I've had, let alone the length in which I worked them. I'd rather not have to make stuff up.

Where have you been? Did you just one day decide to "check out" and finally returned?

Anyway, you should remember where you worked. They can probably help you with the details.

Social Security has all of your detailed record of your work history.

Call your social security administrations office they have a complied list of your job history and can tell you how to get this list.

go to socail security website and ask for a list of your ss witholdings they will send you for free a list aof everywhere youve ever worked and how much you made
