Don't worry TOO much about it. It's really not something that should keep you from enjoying your meal and the people with you.
I once dated a girl for a few weeks. We went to dinner with her parents. Her Dad and I talked the whole meal and had a great time. After dinner, in the car, she says "I'm so embarrassed you used my Dad's bread dish!" I dropped her off and never looked back!
Some other tips:
Do not fear the wait staff. Say thanks or smile and nod. They are not there to judge you, they are there to serve you.
If you don't know what to order, say "Everything looks so good, what will you have?" to others at the table.
I always pick a couple of menu options and ask the waiter "I was thinking the glazed pork or the smoked duck....?" They will always help make the right choice.
Eat the bread with your hands, use the little butter knife and park both on the little plate.
Put your napkin on your lap and if you get up to dance place it on your chair seat.
Most importantly have fun!

yes, work from the outside of the setting toward the inside, but since the settings are often wrong, relax and just make sure you don't get caught eating salad with the large fork, or stirring coffee with a tablespoon.
whatever you do, it can't be as bad as my brother, who used to w ank in the restrooms and come back to the table with a b oner. i think it kept my grandmother alive an extra ten years. (in her final years she was known to expose herself at dinner, too). all this is absolutely true, by the way.

Etiquette - Something invented by the Upper classes to make the rest of us feel inferior.

I have always understood that you start from the outside, perhaps soup spoon, fish knife, steak knife, butter knife will be across a small plate on the left. Dessert spoon and fork furthest away, towards the centre of the table. Please DO use the napkin, folded diagonally.
