The same place as when you're using a Quickutz tool, on top of the raised shape. Put the foam on then squeeze in the tool without paper/card the first time you use it.

I'm not sure. It goes on the front if you are using it in a Quickutz machine but it is not necessary to put it on. The function of the foam is just to make it easier to release the die from the machine. I've found sometimes they work better without the foam.
