TRALI - Transfusion Related Acute Lung Injury. It usually happens after the transfusion of plasma or platelets and is believed to be the cause of Antibodies that the donor has causing an inflammatory response in the lungs. I t can also happen after an RBC transfusion, but not as likely. It can be confused with ARDS - but has a much better prognosis. Sometimes the recipient's reactions are so severe that a ventilator is required and the xray films are very white - showing a lot of involvement -0 but because this is related to an immune response of a donated product rather than a response in the recipient - it is limited and often 24 hours to 4 days later the person is much better. There have been some deaths. An important thing to do when TRALI is suspected is to perform an investigation to identify the donor related to the incident so that they do not donate again. Usually donors that can be associated with a TRALI reaction are those that have been transfused themselves or had multiple children. That is why the ARC now asks female donors if they have had multiple pregnancies.
