If you're in the UK a film called Nil by Mouth. It's too much for most people outside of the UK.

Failing that... Irreversible.

'The Grave of the Fireflies' is a Japanese anime about two orphaned children surviving the Second World War. It is very sad and only someone with a heart of stone won't be blubbing by the end.

'Go Toward The Light' is another sad film on my list. It's about an eight-year-old boy who is diagnosed with AIDS and how his parents have to prepare him for death.
