Consulate or Embassy, whichever is closer to you and have jurisdiction over your area, you may apply there - it is one and the same thing. Please make sure that the consulate must be offering Visa Service as well besides other matters.

You're better off applying at a bank. I hear Capital One's Visa has cash back rewards.

It depend on where your at, and from what country you are from. In the USA, you can apply at any post office.
