Terry Gilliam's "Brazil". And I mean the original Director's Cut not the sanitized studio version.
The ending has us believing that the hero of the story has escaped the totalitarian society with his new-found girlfriend in his arms, and going to someplace better. Then it's all shattered when we discover that it's all an illusion in his mind due to the rather crude lobotomy that his best friend, the doctor, had just given him.
Very stark, unnerving ending.

I guess Im just jaded cuz I saw the *surprise twists* to Sixth Sence and The Others right away.
What has to be the greatest surprise in cimematic history *and its Not The Crying Game or Usual Suspects..though both are excellent* is...............................*drumroll*

Darth Vader *To Luke in Empire Strikes Back*: No, I am your father!
WTF?! Ok, now that I think about it we should have seen that something from 5 miles off. But I can remember the entire movie theatre going dead silent with that line.

"A Beautiful Mind" should have been the weirdest twist, but the publicity around the character gave away too much and nearly spoiled it.
I have to admit, I was totally taken in by that film where the kid "sees dead people!" I can't think of the name of it. The kid was Haley Joel Osment and the psychiatrist was, maybe, John Travolta? "The Sixth Sense." That was it.

this might not be weird, but the best twist ever was in Arlington Road when Tim Robbins gets away with being the terrorist and Jeff Daniels take the blame. This might be the worst Hollywood character to ever come out on top.

I thought the Others was a bit predictable. My favorite is The Game starting Michael Douglas. Man, the last part has soo many twists.

The Others is good..... but I have to say The Sixth Sense for the best twist....never saw it coming.
Secret Window with Johnny Dep has a decent twist to it too..check it out.

I'm pretty good at predicting the twists in movies, but the end of Primal Fear with Richard Gere and Ed Norton Jr. blew my mind! I instatly became a fan of Norton because of this performance. You think all along that he is mentally disturbed, but in the end, you discover he's faking it. It was GREAT!

Tideland. I watched it on Encore one nighttime as a results of fact there grew to become into not the rest on. It grew to become into probable the creepiest movie I ever considered. there grew to become into all this weird and wonderful stuff approximately protecting filled lifeless people of their residences and it grew to become into in simple terms elementary feaky. I commonly love frightening video clips yet this grew to become into in simple terms too weird and wonderful. This and puppy Cemetary are 2 video clips i'm going to on no account watch lower back.
