There is a kit that you can get it consists of a cleaner and a glue.
Use a razor to scrape away all the old "gunk" if any.
Use the cleaner to clean both the metal tab and the glass.
Wait for drying.
Apply couple of drops of glue to tab & glass.
Hold metal tab to glass for 30 to 45 seconds .. until glue starts drying.
(This part they don't say .. but i'd wait overnight for the glue to dry).
Put the mirror back onto the tab & screw back into the tab .. For the next tab be careful not to twist the mirror and break the tab off the glass again.
Within few days everything is back to norm.

First you need to go to a local auto parts store: Autozone, Advanced, Napa...etc. Then you need to buy a rear view mirror attaching kit. This kit comes with typically two parts. One part you will put on the windshield where the mirror mounting plate (little metal plate that the mirror slides onto) used to be located. While the other parts gets applied to the mirror mounting plate. Then the metal plate is placed on the windshield where you just applied the first part on the windshield. When these two substances come together they react and form a bond. You should hold the mounting plate on for a couple of minutes (the kit will tell you like a minute but the longer you hold it on the more likely it will be to stay). Then you just slide the mirror over the mounting plate. If this is to confusing read the back of the kit it has instructions that are easy to follow.

Go to an auto parts store, like auto zone, or auto parts and tell them that you need to re-apply your rear view mirror. They will give you some more adhesive to stick it back on.

forget super glue or anything but..
they make a rear view mirror two part adhesive specifically for this application. available at any auto parts store. follow the directions exactly and it will last as long as oem.

Two sided tape from the hardware store will do the trick, it is very sticky and holds pretty well.
Epoxy will also do it, but try not to make a mess...hahaha

They sell little stick pads to put it on with. Any car store or WalMart-like store would carry it. Word to the wise: don't stick it on in direct sunlight...also, I'd give it a chance to stick. I'd put it on in the evening so it can sit overnight.
