Most of the nations in Europe are "1st World" countries because they existed during the Middle Ages and before. China, India and Japan are arguably 1st World countries because they have been around thousands of years, but they are not usually included because they are non-European.
American countries (US, Canada, Mexico, Brazil, etc.) are "2nd World" countries because they were founded by European settlers in the 'New World'. Australia could also be considered a 2nd World country, even though it wasn't in the 'New World' because it was founded about the same time and in similar manner.

We don't call ourselves a 1st world country, do we?
And, as you say, nobody calls themselves 2nd world.
But we call poor, developing nations, or unstable nations 3rd world countries, because we're the 3rd planet from the sun, hence the name 3rd world.
Note, however, that they do not call themselves 3rd world.
