I think they get a package deal, like air fare, free hotel, a couple meals, transportation to the show, and something for the appearance. It works best for the show if they can control things. Otherwise people would always be late, lost, out of money, and full of excuses.

Not sure if they do, but I have seen mostly celebrities with an upcoming movie and/or album who do these talk shows. For example, Jennifer Aniston was on the nightly talk shows before he latest movie "The Break Up" was released.

No, they don't get paid. They are usually on the show to promote their stuff so why would they get paid. Most shows have a policy on not paying their guests.
