From listening to interviews with many porn stars, it is in the dialogue. They are told to keep the dialogue rolling and when they are stumped for something to say moan. Normally the dialogue is nothing more than sex ques, "do this", "do that", and so on. They throw in some interogatives like "Do you like this?", "How does that make you feel?" , and so on. With such a limited range of things to say the moans fill in the dead space.
As for the thought that some times it is real, there are those time that it is and the actresses have no control over the noice they make. Other times the sound may be dubbed in once agin to fill dead space. Awkward silence is a killer in porn. It gives the viewer a second to think should I be watching this or not? If they think too long they stop watching porn. That would be a bad thing for the industry.
Another thought and it has something to do with anatomy is the diaphram, and I am not talking about contraceptives. As the diaphram collapses from the crunching going on in intercourse, it becomes more and more difficult to project yourself so words are harder to come by. It is logical at such moments to moan if only to catch your breath before saying anything.
And oh yes, the men in these films are for the most part well endowed and know exactly what they are doing so they want to make the girl moan. And the girls are for the most part into having sex, possibly multi-orgasmic, but definitely into what they are doing. They have to be to go at it time and time again so that the director can get his shots. If you think that they are doing this non stop you are not watching the outtakes. Most porns are cut and spliced together from hours of footage which starts and stops for any number of reasons. And "No" the men do not keep their erections through out. Quite often they have to be "fluffed", and quite often the girls are "fluffed" before the filming.
There goes all the secrets.
