From the pacific islands. The occupants of the islands supposedly had sex in many different positions. Then the (almost puritanical) Christian missionarys arrived and were horrified by this fact of life. Advocating the one position, woman on the bottom-man on the top, became a by-word for the missionarys.

the missionaries found that the non believers and pagans of the world were using all kinds of VILE ways to have sex. So they in there infinite wisdom re-educated all the savages to have sex like a missionary. Looking into each others eyes and not seeing what was happening. (They were not allowed to view their pleasure). Hense the term. Now for the love of god don't ever use this position. Its uncomfortable and not very romantic. It originated in the middle ages when the church was the ultimate authority on all matters of morality (including sexual morality).
The idea that the woman should be positioned below the man is supposed to symbolise the relationship between god, man, and woman. In other words, god is above man and man is above woman. Of course, at the time, sex was only sanctioned as a means to procreate, not as an act of pleasure.
I read this in a book called "Women in the Middle Ages" .. can't remember the author.
