I just went through the whole process 4 months ago. I established a LLC. For corporation, the process is roughly the same. You can use those service to incorporate. The service that I used was incorporatefast.com.

Get a good CPA and either they can do it for you or just go online to irs.gov and click businesses. It is 1 page form that you fill out and send to the secretary of state with $50 and that is it. To get a tax ID # you should be able to do that on the same website.

Before you incorporate, check with a good CPA. If you want to do it yourself, go to the website the previous poster gave you.

Unless you really know what you are doing, I would use a reliable CPA.
