This is Psychology. Not Science or math. But I'll answer it anyway.
Psychology says that what you see, is only a fraction of what's really going on INSIDE THE MAN! SAME with a WOMAN! What we SEE is ONLY what we PERCEIVE it to be. Sometimes, it's different from the real world. THAT'S PROBABLY why nations WAR. The US says it's THIS, someone ELSE says it's THAT!! And someone ELSE says, "I'LL SHOW you what it is!!"

While the madman has a dream, he is suddenly awaken, giving him reason to go mad because he lost his dream. And he can't be expected to dream on it anymore by shutting his eyes and throwing his head to the pillow. It doesn't work that way!!! Once awaken in the middle of the night, it's hard to go back to sleep and find the dream again.
Perhaps the madman is a light sleeper :)
The point is, a madman has his "regular" dreams, the unique and creative thought processes that only the back of his eyelids could describe. Perhaps it was a world, an invention, an innovation, or some really random and obscure images thrown together.
In another context: When one is angry, he can not sleep. It's hard to sleep after some disturbance has ocurred... he, the madman, runs the angry images and thoughts through his mind, the dream, while he is lying in bed and contemplates over those thoughts continuously.
