How do you correctly address a letter?

I don't think it matters, but I'd put the PO box on top

PO BOX 555 (all caps, no punctuation)
123 BLAH HWY (all caps)
NEW YORK NY 123456-7890 (all caps, no punctuation, and zip+4)

Go to www.usps.com and look up a zip code - it will give you the zip+4 and give you the exact punctuation, capitalization, and spacing required.

Technically, if you have both a PO Box and a street address, you put the street address first.
