As long as you want,but be aware; long winded ones tend to lack impact I think the chorus or hook should be where one would want to achieve maximum impact.

It doesn't really matter how many lines it is or how long it is. As long as you repeat it after every verse.

It does matter, but it depends on so many variables: the melody, the lyrics, the meter, the tempo, and most importantly, how do you want your song to sound?
