It's not necessary, because if you filled out an application for the company, you most likely had to put the phone number of all the companies that you worked for on the application. You can put the phone number on your resume if you want to, but it's not required.

No. Do not list the phone numbers on your resume. It is commonly accepted to put a statement on the bottom of your resume stating "references available upon request". Even then, only give out phone numbers of people at the company that you know will give you a good reference. Also, it is a good idea to give your references notice that a potential employer may be calling them for a reference.

It doesn't matter. Because most companies are going to have you fill out an application anyway and you have to put that stuff there. The phone numbers on the resume are primarily for you. I list my references and prior companies on a separate sheet for quick reference when I'm feeling out the application.

No, not their address either.
Just the name and city, and state. It should look like this for example:
Sales Associate
Johnson Volkswagen, Sacramento, CA
- Employee of the month
- Top sales associate 3 years consecutively
- Blah, blah, blah
Your job title, the line below is the company, then bullets of responsibilities.
Phone and address are for the application form and the references which are on a separate sheet.

When you're listing employment history, you don't have to give it unless there's a specific area for a phone number. You should supply phone numbers with your references, who I suspect will mostly be former coworkers of yours.
