I don't think anyone can say for sure but I think the ones who are very involved in their cause not just taking pictures with poor people or things like that are the ones who really care.
One who seems to really care is Edward Norton, he does a lot of good thing ans seems very passionate and smart about what he does and doesn't really publicize it, he just does it.

I think the ones who TRULY care are the ones who do it without feeling the need to go on TV and talk about how great they are for doing it...they also don't ask ME for money.
Bill Gates is the best example of this I can think of.
