Most tire shops offer rotate and balance specials. Generally tires do not need re-balancing unless worn unevenly due to alignment or other issues. Most vibrations are due to this and not balance.Vibration when using the brakes is not balance either but due to brake system components out of round or uneven.
Most shops will do a road test and let you know what it is before spending your money.

5 to 10 dollars a tire. But if you buy a set of 4, sometimes if your a good bargainer you can get the company to throw it in for free. But only do the bargaining after you agreed to the price per tire otherwise they will add it to the cost of the tires

I think it is about 30 dollars. I get my roated for free at the place where I bought my tires. You should check where you bought your tires or the dealership where you bought your car from.
