If Madame X is froma traditional family, then chances are she has been sheltered and primed for her role in life as a good wife. I am not sure how her system can cope with being away from her husband, especially as she must haev been very much used to relying totally on him (her husband went to work overseas when her son was 18).
I think she feels very insecure, and she's not insane. She could be worried about playing the role of ideal wife, whether she has succeeded or failed. This can lead to the nightmares, compulsive desire for cleanliness and manipulation as she likely wants people to remain close to her or within her sphere of control.
I think the key to her 'recovery' would be the people around her. How do her relatives treat her? Is she the only one in the extended family to have a husband working overseas? Is she seen as a poor match for her husband, since he 'left'?
People around her can make things better or worse; what she needs is some recognition for what she has actually done - her son is 18 surely she must have helped in bringing him up, and the issue of teh husband going overseas to work resolved. When is he coming back? Is he sending $ back home? Who controls that $ he sends back home?

Not insane. Maybe an obsessive compulsive disorder and possibly an autistic spectrum disorder, probably aspergers.
The fear and selfishness could also have been encouraged by the lack of education and understanding and if she was the best-looking girl in the village, I would guess she had a lot of male attention which would have probably caused a big ego boost.
All of those may have contributed to the severity of the problem but the problems would have already been there.
She is not insane.

i highly doubt X is insane. she just needs help.
*what does she have nightmares about? maybe she has repressed unpleasant memories in her subconscious that only surface in dreams.
