Real all the way. Fake boobs are too artificial looking and feeling... they just don't move right. I'd just stick with small breasts since a handful is all you really need anyways. ;-) Plus I've heard from girls with boob jobs that after the surgery it feels like an elaphant is standing on your chest. Also, large breasts can lead to back problems. Well, I could go on and on about boobs, really, but I'll stop now. Heh.
