I hardly ever do except when the service was truly exceptional. And if I do, it will be generous and even then, it is not determined by a percentage or amount. It just depends on how much I liked the service.

After all it is a GRATUITY and why must we feel pressured to tip everytime. And what's up with some restaurants forcibly charging you tip. Its more like a tax which you don't want to pay but you are forced to pay. It should be called a tax.

I usually tip 10% - 20% depending on the service.
Sometimes my tips are influenced on the different laws of the areas I am in, for example in Washington State waiters/waitresses are paid minimum wage and tips are on top of that, while in many other places they can be paid less than minimum wage using the tip to make up the difference.

i don't deal with percentages... i just give tip flat-rated which is 2 dollars. if i'm with a partner, we tip 2 a person so that's 4 bucks; $2 per person. if the service was great, an extra dollar is added. if the service was terrible, i tend to return the favor and tip them loose change.
