You dont ask him on the phone first off thats well to put it bluntly the shitty way of doing things. You ask someone face to face its a lot better that way and you just say whats on your mind, like obviously you've noticed certain things about this person and thats what makes them unique and why you like them. Just look inside you and you will get your answer, dont hide behind a phone.

Tell him that your really interested in him and that you think hes a cool guy and then just go for it and ask him out and if he says no better luck next time!!

Well if you wanna make it a group date just be like "Hey what are you doing this weekend? Me and a couple friends are thinking about (fill in the blank) You wanna come along?" Or if this is supposed to be a single date just be like "Hey i think it'd be cool if we did something together this weekend, since we never hang out. What do u think?"

well girlie girl first you dont need to "ask him out"
You just say to him "what are you doing?" then tell him that you are going to go down to the rollerrink or mall or movies or wereever kids go these days and say "do you want to meet down there?"
If he says yes and does meet you there then its basically a date, if he doesnt give him one more try to meet somewhere again in a few days and take it from there!

That you like him and you would like to go out and have some fun, that you think it would be cool to date or hang out. He will most likely be flattered and accept.
If he doesn't, don't freak, just try another guy.
