Does that mean you twist the stick to control the rudders in a flight simulator game. It seems like it would be hard to control the pitch, roll, AND yaw with the same stick.

Yes that is correct, it is for rudder. You usually don't need to apply a lot of rudder, it is to make the turn a smoother turn or more coordinated. You would have to have some pilot experience to really know what I mean. I have used that control also in Mech Warrior style games to control the Torso movement of the Mech you are driving.

I have a joystick with a Z axis and pedals. I prefer using pedals, though the Z axis is nice for when There's something blocking the pedals.
CH makes excellent pedals/yokes/throttles, though they're a bit expensive. You can probably find a twist axis joystick pretty cheap though.
