Call your local "County" services building. They are usually the ones that can set you up with that.

That depends on what state you live in and what their laws are regarding the matter. You might contact Child Protective Services (that's who regulates in-home daycares here in Texas) or the Department of Human Services in your state to see what you'll need to do for the license.
