I was going to say, tell the both of them they better say something before you do, it'll look a lot worse if they don't come out and say it.
However, then I decided against that...Number 1, you are the best friend (BFF) , so you must tell her. Suggest asking the BFF that you know something about her sister and her. Tell her you think it'll be better if she hears it from the sister, but if she wants to know right away, you are willing to tell her! You don't owe any loyalty to the sister or the boyfriend, only to the BFF. Do it, and do it right away. She's already gonna lose a LOT of loyalty/trust from losing faith in her sis and boyfriend, she doesn't need to add you to the list...

Hmmm...now there is a twister about a sister if I ever heard one....I think it is best to keep it confidential otherwise you will open the storm gates and a lot of hurt will fall out......

I think it'd be best if you told her boyfriend that HE needs to tell her.
If he won't, then someone needs to, i.e., you.
