Probably Jack "Splat" Lambert, of Pittsburg Steeler "Steel Curtain" fame. He epitomized the gritty, mean, crazy, hard working linebacker that linbackers today could only dream of being. He was told he was to small to play professional football (Jack didn't lift weights due to the fact that he had an accident lifting weights in college leading to the loss of his front teeth). One of seven linebackers elected to the NFL's 75th annaversay all time team, former defensive rookie of the year, 9 consecutive pro-bowls (including his rookie year), 1979 defensive player of the year, and 4 Super Bowl rings.
Close in the running? Butkus and Bednarik, but Jack Splat has got them for sure.

There are a lot of truly great middle linebackers in NFL history. Dick Butkus stands out as the best. He was a ferocious tackler. He could sack the QB or drop back in coverage.
Others worth mentioning include:
Joe Schmidt. He was part of the great Lions teams of the 1950's.
Ray Nitschke. He was the heart of the Packer defense during the Lombardi era.
Nick Buoniconti. The QB of the "No-Name Defense" of the great Dolphin teams of the 1970's.
Jack Lambert. A fierce, old-school linebacker from the Steel Curtain team of the 1970's.
Willie Lanier. From the Chiefs of the late 60's early 70's great quickness and agility.
Mike Singletary. The Bears of the 1980's. Very smart, very tough and a great intimidating stare.
Harry Carson. From the Giants of teh 1970's and 1980's. Also very smart and as sure and fundamentally sound a tackler as I ever saw.

It's not a popular answer, but I agree with Rob (above answer.) In my eyes, Mike Singletary is the best mlb to ever play. He wasn't known for being as vicious as Butkus, Lambert, and Nitschke, but he was exceptional in every aspect, and he had incredible instincts. Back in the 80's and even towards the early 90's, you rarely saw a play that he wasn't involved in. Even if he wasn't in on the tackle, he was never more than two yards removed from a play. Run or pass, he always seemed to know where the ball was going, and he knocked the piss out of whoever happened to be holding it.
And by the way, Ray Lewis should not even be a consideration. He's a good LB, maybe the best in recent (and i mean very, very recent) years, but he's not even close to be the GOAT.

By not knowing that MLB stands for Middle Linebacker, I would have to disregard the answers of those who thought MLB stood for Major League Baseball.....especially the one who said Derrick Thomas. The best MIDDLE LINEBACKER of all-time would be Mike Singletary.

Butkus. No one else comes close.
And LT was an outside linebacker, not a middle linebacker.
