your brake rotors are warped due to over heating . thi may happen if you do a lot of stop and go driving or driving where there are a lot of hills . the rotors can be resurfaced withought having to get new ones if they are within specs

my guess is that your "rotors" need to be turned....there is a good possibility that you might need brake shoes as well....your rotors got out of round probably because the shoes are or were down to the metal.....did you recently maybe had new brake shoes put on and the rotors not turned??
