I suspect yu maybe having an issue with your power steering pump. Yes, you should have it looked at, as replacement cost can be substancial, but if only needing fluid, that would cost a few dollars!

Steering wheel vibrations are usually caused by your front wheel/wheels needing to be balanced. It can also be caused by bad tires, a bad front suspension component, or a problem with the hydraulic steering pump or other steering component.
Take the car to a shop and have it looked at.

Yes, it's really bad, and you should have it checked by a professional immediately. It sounds as if something in the mechanical part of the steering is going bad. If it's loose enough that you can feel it, especially regularly, it's definitely in bad shape. SLOWLY drive to a mechanic.

Check the air pressure in your tires, or have it checked. A lot places will do this for free. Have them look at your tires as well and make sure they are ok. Your tires might also need to be rotated because they do wear on one side. If none of this works try having an alignment. When my did this it was because the air pressure was not correct.
