This goes back to the forties when a manchester city player called jimmy sanderson put the ball through the legs of a wolves player called meg nuttall, as he did this he shouted "meg" and through the following decades people told this story, but it was only in the 1990's that other players started shouting "nutmeg", which obviously used the first part of nuttall's surname together with his first name and the word which was shouted originally, I think it is a case of things slowly changing over the years and it just stuck.
