For NRIs (Non Returning Indians) all fears are valid - presumably. Scores of my personal acquaintances (various nationalities) enjoy trips to India without having a dispensary (medical store) in their body. If you are sick otherwise, you must take due medical precautions.

I am not sure that anything is really required, but I do know what is strongly recommended. When I went 6 months ago, I was told to make sure my Hep A and B and Tetanus were up to date. I also received a Polio shot, as I had not already been vaccinated for it. Other than that, because I was going to other parts of India, I had anti-malarial drugs. I think I might have also gotten Typhoid, but I am not sure. It is recommend by the CDC. They also recommend some other stuff depending on what sort of areas you will be going to. If you are just in the city, you probably need less.

I do not believe there are any special requirements for Indians returning to the country (I assume that is what nris means).

As such nothing specifically required but yes if u r prone to any disease or sort of infection thn must take care in advance. Just take care of water (always prefer bottled water) and avoid roadside eatables.
