Have you ever considered the possibility of career counseling?
You never did say what your career is, but if you aren't satisfied with it then it doesn't much matter. If I were in your position, and I have been twice, what I would do is consider what career is best suited to what I'm now expecting out of life. You never did say what specifically does not satisfy you about your career, but if you examine what isn't right about it for you, and then search for alternative paths that can give you what you're missing in your current position.
Clear away the fog that has stopped you from making a decision by assessing your situation objectively in terms of what you need, want, and would like to have in a career. Ask yourself, what is important to me in a career? What is important to you in life? What do you have to have to feel satisfied?
Create a plan that allows you to make the transition from one career to another smoothly. What can you do today that will ensure you get the results that you want? Appraise your situation in terms of what you need to make what you want happen. If you lack resources, plan on acquiring them before you try to switch careers.
Chronic indecision isn't a good frame of mind to have, try to be more flexible with the way you think about things. It's time to come down to the realization that now is the time is not your friend. Waste it and you never get it back. Move into action with a plan, and be bold.

Ask yourself if you had the power to choose a job that you would like to do over all others what would it be? Next, try to figure out what steps are needed to get there. I work in IT and I've changed jobs every 4-5 years. I liked my jobs and the companies I worked for but I lreally ove to travel. I figured if I could find an IT job that also let me travel internationally I'd be in heaven. So I just kept researching and tried to match my skills in IT with a company that had an international presence. It finally happened for me in January of this year. Now I travel overseas for my company doing what I love. I'm 51 so I don't think it's ever too late.
As for letting your family down, I think you need to realize that YOU are the one ultimately that needs to live with your choices and so it's better for you to be happy and fulfilled and make that happen first. Once you are happy and fulfilled THAT is what will have the best impact on your family.

ask yourself what makes you happy... then figure out how to let that be the center of your universe... if you love chocolate, then work int he chocolate industry... do you like watching tv?... do something in media... it's never too late to start anything... and if you ahve a family that supports you, then you've got it made... i love to travel, so i work my butt off for half the year and i use my savings to travel for the next 6 months... it's not practical for retirement and all that, but i would rather be living this way than being stuck in some office waiting for retirement... travel makes me happy so i do whatever i can to do it... and that makes all the jobs worth it to me... maybe you haven't had a lot of experience in differnt fields, well, enough to help you decide what you would like to do... don't think of one job as the end of everything... at 46 you should enjoy your life and you have a lot of years left to build a career... a friend of mine became a piano instructor at age 42 because she never thought of it as a career before... she's the happiest person i know...
i hope you find something that will fulfill you... because being happy, joyful, feeling useful is so important... listen to some music and let the melody help you to clear your mind... that's the first step in finding out what you want to do...=)
i can recommend beethoven symphony #7, second movement... or brahms symphony #3 third movement... they are the most emotionally intense pieces i have ever heard...

a job is different than a career.
choosing a DIFFERENT career requires different training. maintain a secure job and then get education at a community college, like night courses. take different things that interest you, not just one. once you find something that inspires you, make sure there are enough jobs in that field for an entry level worker to have a chance (like teaching or nursing). also, entrepreneurs come in all ages, but that takes even more knowledge. if you have a good idea, though, you could become very wealthy.
take some tests that asses your interests and talents and start from there.

Since you have no sense of direction I suggest you make due with a career that provides for your family and find yourself a hobby or interest that because of your career affords you to do both...that way you interests are met and you can have some sort of appreciation for your job for being able to do both...enjoy what you have...not what you don't have.
