this depends on what type of email you are using in outlook if you select tolls then e-mail accounts the information is there

it's normally smtp. and pop3...
the password is the password you use to log in to your account? contact your ISP.. or it a fre one like hotmail/yahoo/gmail.. look in the help guides.
also most ISPs allows you to recover lost passwords, (Resetting it)
