I would ask how first because the how is objective and more or less constant whereas the why is subjective and can change from person to person.

The why for sure.
The absolute why will detail the how anyway.

Why do you want to clean the house?
Each of your answer will lead to a new set of questions.
When you have answered all the questions, you will know exactly how you want to clean the house.

See, how serves the purpose of why.
Why defines the purpose, or the problem.
Better the definition of why, better methods (how) shall be implemented.

And in order to detail, the "How", you have to find
Who, when, where, within in what time limits, with what skills, and technology, concerning who etc.,

oh, I read you question again and it looks like you might want to know in case of an Incident, (accident etc.,) that has already happened.
Actually , we ask the question how did it happen?
But we really mean to ask why did it happen? The reason or the cause of the incident. Now, how it happened (the drama) is worth following up with depending on how interesting it is or how much time we have to know the details of how.

I would most likely ask HOW? Then I would want to know why, but every situation depends and is different.

Why. I am constantly asking why about everything, becuase I want to know why. Who cares how it happened? It just did, and I want to know why.
