Fellowships (and teaching assistantships, and research assistantships) are offered to top applicants by each PhD program.

Here's how it works:

Students apply to PhD programs in each PhD-granting department of a university. The graduate admissions committee in each program then selects the candidates to which they will offer admission. Each program then has the ability to award a certain number of assistantships to its top candidates, and to nominate its VERY top candidates for fellowships. The applications for fellowships are then sent to a university-wide fellowship selection committee, and this committee then chooses which nominated candidates will receive fellowships.
