He's alive, but the whole movie takes place while he's dying on the battlefield.

He is alive in the movie and through the movie I'm sure he wished he were dead. But he is living and continued to live at the end. The LSD experiments that the US Gov did on solders back then did kill a majority of the troops that were the test subjects. But a few are said to still be out there!

I think Jacob was dead because he saw his dead son waiting for him. Yes, you can argue he was alive, but I truly think he was dead. The demons he saw during the movie represented his spirit going through Hell
