I'm starting a delivery service for furniture and other household items. I'm trying to make a comment card to get feedbac from my clients.

Was the delivery on time?
Did the product arrive in perfect condition?
Was the delivery person professional and polite?
How would you rate the delivery cost?
How would you compare this delivery to competitor businesses?
How was your overall service?
Would you use this service again?
Would you recommend this service to friends and family?

I would also print the card on a pre-postage paid card and offer a discount or small gift for completing the card.

1. Was the service completed in a timely manner?
2. Were the workers friendly and helpful?
3. Were the goods delivered to you in good condition?
4. Were all items that you ordered included in this delivery?
