Regular and surprise inspections are carried out at a minimum of annually for all restaraunts, anywhere food is sold as well as a variety of other public venues.
In other words, someone would know or be turned in quite easily for such practices.
So don't worry, when you go to a burger joint, that's what you're getting. Fine dining? That's what you're getting. And if you find that not to be the case at anytime, call your local health department!

Ha ha! You have to trust the restaurants or you cook your own meal! There is only one such case I've heard of in Singapore that happened long time ago. A murdered Indian man was butchered and cooked in a pot of curry and fed to a dog. But hey, the woman who did that isn't a chef! My point is, we are not inhuman.
