One of the best questions I've seen.
I've never felt like the thank-yous I give are good enough.
It obviously depends on the person and the situation.
For everyone: the words thank-you (many don't know what to say or forget to do so)
For my wife: flowers/intimacy
For my kids a hug/time/attention
For coworkers a card
For a boss ( just the words, a smile and hard work )

It depends on the action that deserves the thanks and the relationship of that person to you.
If the person is a boss, superior, etc., a verbal thank you and written note is appropriate.
If the person is someone who is close to you a verbal thanks and a hug is appropriate as might be a small gift.
If the person really went out of their way to do whatever you are thanking them for a larger gift might be appropriate. Most importantly - do not do anything that might embarrass the other person or make them feel like they now owe you back something.
A written note expressing appreciation is always a nice remembrance for the other person. I still have cards and notes sent to me from students, clients, and friends. Whenever I stop and wonder why I am doing the kind of things that I do I read one of those notes and it reminds me why I do them.

I always tell someone thanks and then add something about how wonderful they were to help me, or what a great idea they had or something like that. I find many appreciate a smile to go with it.
And when you do say thank you to someone .. tell them why you are grateful.. Such as "Thanks for that great recipe.. I loved it.."
Or "thanks for your advice, it really is so wonderful of you to help me.." ect.
I also forgot to add that the tone of your voice should reflect how you feel..
