In order for an index to work you have to add at least one index entry.
Start at the beginning of the document. Say the first heading. Move the cursor to the beginning or the end of the heading. You will have to try both to see what happens.

Now Press ALT+SHIFT+X.
A window will appear where you can enter the index entry under Main Entry.
Click on OK.

The text you type will appear in the document between curly brackets. This is non printable text.

Now insert your index and you are done. The problem is that for a large document you will have a lot of work to do.
