No, it's used to stress that what they are about to tell you is strictly their opinion, which may go against popular opinion. For example, one might say "Everyone seems to like Dave Matthews Band, but to be totally honest, I think they're awful."

No one is totaly honest every one has lied once in there life. And it depends on what kind of person is saying that. i can trust my freind with nearly anything.
