To get a free upgrade is very rare. The best way is to ask very nicely at check-in and perhaps have a reason for wanting the upgrade besides I want a nicer room. They are more likely to give discounted upgrades when they are not full. Just ask at check how much it would cost to upgrade your room. You would be surprised. I was upgraded from standard room to deluxe water view for only $10 per night.

They certainly do. When we went to WDW last December I booked two of the cheapest rooms at the Wilderness Lodge (still $299 per night). When we arrived I asked if anything better was available and they put us in their top of the line pool view room ($479 per night) at NO EXTRA CHARGE!!! Walt must have rolled over in his grave.

rarely. They did for us on our honeymoon, but haven't since. It depends on the time of year as well, if you're going during a peak time, when they're fuller, they won't be as inclined to do it. But if you're going in the middle of September, January or between Thanksgiving and Christmas, you might luck out.
I love the mousesavers site for ways to save $$ at Disney.

Yes, they do. They don't always do it and it can be rare, but when they need to or can, they will upgrade you. It never hurts to ask when you arrive if there are any upgrades available.
