Is this something that I write on tablet paper and both parties sign? OR Is this a legal form that I need to obtain from the prothonatary? Basically, this is a "how do I obtain a written stipulation to vacate?" question.

This would be in reference to a "no contact order" that neither myself or my fiance wanted. It was ordered against our wishes and I was told to file a written stipulation to vacate the order but not told how to do it.

you can put it down written on paper but you need to have the correct court caption and case number like it is on your court doc's

You need to provided more information the context of your question. A stipulation to vacate is usually done in the context of settling an eviction lawsuit. If the renter doesn't comply, the stipulation is usually enforceable as a court judgment for possession of the rental property.
