Go to your local dive shop or contact the certifying agency. I can't help you anymore than that, you don't even mention if it was PADI, NAUI, BSAC, IANTD or what.

I agree with ScubaBob - check the website for your certifying agency - they usually have a procedure to go through to replace lost cards.
